Most people assume that germs and genes cause disease. The germ theory has brought us a long way, and genetic theory promises to take us even further. But there is still a mystery surrounding why certain people get sick while others don't.

For example, studies show that if cold virus is placed directly into a person's nose, the chance of getting a cold is about 1 in 8; being exposed to chill, damp, or a draft doesn't increase these odds. Also, when the Black Death wiped out a third of Europe's population in the 14th century, no one knows why the other two-thirds, who were certainly exposed, didn't die.

Every day each of us inhales or ingests enough germs to cause a variety of diseases we never contract. Some sort of "control by the host" seems to be at work. This refers to the body's ability to live with disease-causing agents without getting sick. Germs aren't the only factor. Statistics show that severely ill people often wait until a significant date has passed, such as Christmas or their birthday, before suddenly dying. Studies going back to the Korean War showed that young soldiers in their early twenties had serious blockage of their coronary arteries, yet the disease doesn't show up until middle age. Not everyone exposed to HIV contracts the virus, and in a few rare instances, those with AIDS have reversed their viral status form positive t negative.

Why, then, would you or I get sick when someone else equally at risk doesn't?

The best way to get sick is to suffer from as many of the following conditions as possible:

· Unsanitary conditions: massive exposure to germs remains a major factor

· Being poor: poverty degrades life on all fronts, including health.

· High stress: physical and psychological stress damages the immune system.

· Depression and anxiety: untreated psychological disorders weaken resistance to a wide range of diseases, perhaps even cancer

· Lack of coping mechanisms: stress by itself is a negative factor, but the inability to bounce back form it is more important.

· Lack of control, victimization: all stresses become much worse if you feel that you have no control over your own life.

· Inertia, sedentary lifestyle: if you are inactive and have no outside interests, you chance of getting sick rises sharply

· Feeling alone and unloved: emotional deprivation is as unhealthy as deprivation of good food.

· Sudden loss: the sudden loss of a job or spouse, a reversal in finances, or finding yourself in the midst of a war or natural disaster all constitute a state of loss and lead to higher risk of getting sick.

· Growing old: once considered a major cause of illness, aging is now known not to be a direct cause. Being healthy into your eighties should be your expectation, but if you neglect yourself in old age, the body becomes vastly more susceptible to disease.

None of these factors comes as a huge surprise, since public health officials have drummed into us that most illness in modern society is a "lifestyle disease" born of stress, lack of exercise, and other factors external to germs. But I think most people still assume that being fat, for example, is worse for you than stress, which certainly isn't the case. Outside of diabetes and joint problems, it's hard to find a serious link between moderate overweight and any disorder, while stress and its offshoots are major risks. They exaggerate the effect of aging. Yet in the absence of high blood pressure and artery disease, most people will live a very long time, probably in good health until they contract their final illness.

But the mystery of who specifically gets sick remains unsolved, in part because there are subtle factors that few experts have adequately examined.

· Some people get sick because they expect to.

· Some people get sick, or sicker, after they are diagnosed with a disease.

· Disease brings certain benefits, known as "secondary gain," that make it positive. The classic example is a child who pretends to be sick in order to get more love and attention, but adults find secondary gains of their own, such as not having to take responsibility for their lives or finding an escape from a situation they can't cope with.

· Some people get sick because they want to give up, or even die.

· Some people have nothing better to do than to get sick.

Three different ways infectious disease can be spread through direct contact are:

· Person to person. The most common way for infectious disease to spread is through the direct transfer of bacteria, viruses or other germs from one person to another. This can occur when an individual with the bacterium or virus touches, coughs on or kisses someone who isn't infected. These germs can also spread through the exchange of body fluids from sexual contact or a blood transfusion.

· Animal to person. Your household pet might seem harmless, but pets can carry many germs. Being bitten or scratched by an infected animal can make you sick and, in extreme circumstances, could even cause death. Handling animal waste can be hazardous, too. For example, you can acquire a toxoplasmosis infection by scooping your cat's litter box, particularly if you're pregnant.

· Mother to unborn child. A pregnant woman may pass germs that cause infectious diseases to her unborn baby. Germs can pass through the placenta, as is the case of the AIDS virus and the toxoplasmosis parasite. Or germs could spread during labor and delivery, as is the case for a mother infected with group B streptococcus.

Indirect contact

· Disease-causing organisms can also be passed along by indirect contact. Many germs can linger on an inanimate object, such as a tabletop, doorknob or faucet handle. When you touch the same doorknob grasped by someone ill with the flu or a cold, for example, you can pick up the germs he or she left behind. If you then touch your eyes, mouth or nose before washing your hands, you may become infected. Some infections occur from organisms that naturally live in the environment but are not passed from person to person. Examples include fungal infections like histoplasmosis or blastomycosis, as well as bacterial infections such as anthrax. Infectious diseases spread through the air

· Droplet transmission. When you cough or sneeze, you expel droplets into the air around you. When you're sick with a cold or the flu — or any number of other illnesses — these droplets contain the germ that caused your illness. Spread of infectious disease in this manner is called droplet spread or droplet transmission.

· Droplets travel only about three feet because they're usually too large to stay suspended in the air for a long time. However, if a droplet from an infected person comes in contact with your eyes, nose or mouth, you may soon experience symptoms of the illness. Crowded, indoor environments may promote the chances of droplet transmission — which may explain the increase in respiratory infections in the winter months.

· Particle transmission. Some disease-causing germs travel through the air in particles considerably smaller than droplets. These tiny particles remain suspended in the air for extended periods of time and can travel in air currents. If you breathe in an airborne virus, bacterium or other germ, you may become infected and show signs and symptoms of the disease. Colds caused by viruses, influenza and tuberculosis are a few types of infectious diseases usually spread through the air, in both particle and droplet forms.

· Pay special attention to cleaning the 'hot zones' in your home. These include the kitchen and bathroom — two rooms that can have high concentrations of bacteria and other infectious agents.

· Practice safe sex. Use condoms if you or your partner has a history of sexually transmitted diseases or high-risk behavior — or abstain altogether.

· Don't share personal items. Use your own toothbrush, comb and razor blade. Avoid sharing drinking glasses or dining utensils.

· Travel wisely. Don't fly when you're ill.

·With so many people confined to a small area, you may infect other passengers on the plane. And your trip won't be comfortable, either. Depending on where your travels take you, talk to your doctor about any special immunizations you may need.

· Keep your pets healthy. Bring your pet to a veterinarian for regular care and vaccinations. Keep your pet's living area clean. With a little common sense and the proper precautions, you can avoid infectious diseases and keep from spreading them.

What is H1N1?

Influenza A virus subtype H1N1, also known as A(H1N1), is a subtype of influenzavirus A and the most common cause of influenza (flu) in humans. Some strains of H1N1 are endemic in humans and cause a small fraction of all influenza-like illness and a large fraction of all seasonal influenza. H1N1 strains caused roughly half of all human flu infections in 2006.[1] Other strains of H1N1 are endemic in pigs (swine influenza) and in birds (avian influenza).

In June 2009, WHO declared that flu due to a new strain of swine-origin H1N1 was responsible for the 2009 flu pandemic. This strain is commonly called "swine flu".

When the immune system is fighting pathogens, cytokines signal immune cells such as T-cells and macrophages to travel to the site of infection. In addition, cytokines activate those cells, stimulating them to produce more cytokines. Normally, this feedback loop is kept in check by the body. However, in some instances, the reaction becomes uncontrolled, and too many immune cells are activated in a single place. The precise reason for this is not entirely understood but may be caused by an exaggerated response when the immune system encounters a new and highly pathogenic invader. Cytokine storms have potential to do significant damage to body tissues and organs. If a cytokine storm occurs in the lungs, for example, fluids and immune cells such as macrophages may accumulate and eventually block off the airways, potentially resulting in death.

Influenza A virus strains are categorized according to two proteins found on the surface of the virus: hemagglutinin (H) and neuraminidase (N). All influenza A viruses contain hemagglutinin and neuraminidase, but the structure of these proteins differ from strain to strain due to rapid genetic mutation in the viral genome.

Influenza A virus strains are assigned an H number and an N number based on which forms of these two proteins the strain contains. There are 16 H and 9 N subtypes known in birds, but only H 1, 2 and 3, and N 1 and 2 are commonly found in humans.

How do people become infected with the virus?
The virus is spread from person-to-person. It is transmitted as easily as the normal seasonal flu and can be passed to other people by exposure to infected droplets expelled by coughing or sneezing that can be inhaled, or that can contaminate hands or surfaces.

There are no known instances of people getting infected by exposure to pigs or other animals.

The place of origin of the virus is unknown.


What are the signs and symptoms of infection?

Signs of influenza A(H1N1) are flu-like, including fever, cough, headache, muscle and joint pain, sore throat and runny nose, and sometimes vomiting and diarrhoea.

But influenza A(H1N1) is a new virus and one to which most people have no or little immunity and, therefore, this virus could cause more infections than are seen with seasonal flu. WHO is working closely with manufacturers to expedite the development of a safe and effective vaccine but it will be some months before it is available.

The new influenza A(H1N1) appears to be as contagious as seasonal influenza, and is spreading fast particularly among young people (from ages 10 to 45). The severity of the disease ranges from very mild symptoms to severe illnesses that can result in death. The majority of people who contract the virus experience the milder disease and recover without antiviral treatment or medical care. Of the more serious cases, more than half of hospitalized people had underlying health conditions or weak immune systems.

Most people experience mild illness and recover at home.

When should someone seek medical care?

A person should seek medical care if they experience shortness of breath or difficulty breathing, or if a fever continues more than three days. For parents with a young child who is ill, seek medical care if a child has fast or labored breathing, continuing fever or convulsions (seizures).

Supportive care at home - resting, drinking plenty of fluids and using a pain reliever for aches - is adequate for recovery in most cases. (A non-aspirin pain reliever should be used by children and young adults because of the risk of Reye's syndrome.)

Seasonal influenza is an acute viral infection caused by an influenza virus.

There are three types of seasonal influenza – A, B and C. Type A influenza viruses are further typed into subtypes according to different kinds and combinations of virus surface proteins. Among many subtypes of influenza A viruses, currently influenza A(H1N1) and A(H3N2) subtypes are circulating among humans. Influenza viruses circulate in every part of the world. Type C influenza cases occur much less frequently than A and B. That is why only influenza A and B viruses are included in seasonal influenza vaccines.

Death from influenza A(H1N1)

Many people also continue to ask why the virus has caused death mainly in adults between ages 20 and 40. At this point, scientists simply do not know. One legitimate possibility has to do with antibody cross-reactivity—the ability of an antibody generated from previous exposure to an antigen to react with a similar antigen on the new strain of virus. Older people, because they have been alive longer and have been exposed to more antigens than younger individuals, may possess a sort of “background immunity.” Because younger people do not have this immunity, they may be more susceptible to severe illness.

There also exists an aberrant immune response called a cytokine storm, in which the immune system of a healthy individual severely overreacts to a viral infection. In the case of influenza, cytokine storm is suspected to cause severe inflammation in the epithelial cells of the lungs, which can lead to death. However, little is known about this immune response in humans as it relates to influenza, and there is no evidence that this is what caused the deaths from influenza A(H1N1) in Mexico.

ANTIBODY

Biochemistry also called immunoglobulin

A protective protein produced by the immune system in response to the presence of a foreign substance, called an antigen. Antibodies recognize and latch onto antigens in order to remove them from the body. A wide range of substances are regarded by the body as antigens, including disease-causing organisms and toxic materials such as insect venom.

Antigen

Foreign substance that, when introduced into the body, is capable of stimulating an immune response, specifically activating lymphocytes, which are the body’s infection-fighting white blood cells. Virtually any large foreign molecule can act as an antigen, including those contained in bacteria, viruses, protozoa, helminths, foods, snake venoms, egg white, serum components, red blood cells, and other cells and tissues of various species, including humans. An antigen that induces an immune response—i.e., stimulates the lymphocytes to produce antibody or to attack the antigen directly—is called an immunogen.

Cytokine

Any of a group of small, short-lived proteins that are released by one cell to regulate the function of another cell, thereby serving as intercellular chemical messengers. Cytokines effect changes in cellular behaviour that are important in a number of physiological processes, including reproduction, growth and development, and injury repair. However, they are probably best known for the roles they play in the immune system’s defense against disease-causing organisms.

Immune System

Cells, cell products, organs, and structures of the body involved in the detection and destruction of foreign invaders, such as bacteria, viruses, and cancer cells.

Immunity is based on the system’s ability to launch a defense against such invaders. For the system to function properly, it must be able to distinguish between the material of its own body (self) and material that originates outside of it (nonself). Failure to make this distinction can result in autoimmune diseases. An exaggerated or inappropriate response by the immune system to nonharmful substances (e.g., pollen, animal dander) can result in allergies. The system’s principal cells include lymphocytes that recognize antigens and related accessory cells (such as phagocytic macrophages, which engulf and destroy foreign material). Lymphocytes arise in the bone marrow from stem cells, with T lymphocytes (T cells) migrating to the thymus to mature and B lymphocytes (B cells) maturing in the bone marrow. Mature lymphocytes enter the bloodstream, and many become lodged, along with accessory cells, in various body tissues, including the spleen, lymph nodes, tonsils, and intestinal lining. Organs or tissues containing such concentrations are termed lymphoid. Within these organs and tissues the lymphocytes are confined within a delicate network of connective tissue that channels them so they come into contact with antigens. T cells and B cells can mature and multiply further in lymphoid tissue when suitably stimulated. Fluid (lymph) draining from lymphoid tissues is conveyed to the blood through lymphatic vessels. Lymph nodes distributed along these vessels filter the lymph, exposing macrophages and lymphocytes contained within to any antigen present. The spleen plays a similar role, sampling the blood for the presence of antigens. The capability of lymphocytes to pass between lymphoid tissue, the blood, and lymph is an important element in the system’s functioning.

Immunity from disease is actually conferred by two cooperative defense systems, called nonspecific, innate immunity and specific, acquired immunity. The lymphocytes regulate or participate in the acquired immunity to foreign cells and antigens. Signs and symptoms

Seasonal influenza is characterized by a sudden onset of high fever, cough (usually dry), headache, muscle and joint pain, severe malaise (feeling unwell), sore throat and runny nose. Most people recover from fever and other symptoms within a week without requiring medical attention. But influenza can cause severe illness or death in people at high risk (see below). The time from infection to illness, known as the incubation period, is about two days.

Who is at risk?

Yearly influenza epidemics can seriously affect all age groups, but the highest risk of complications occur among children younger than age two, adults age 65 or older, and people of any age with certain medical conditions, such as chronic heart, lung, kidney, liver, blood or metabolic diseases (such as diabetes), or weakened immune systems.

Treatment

Antiviral drugs for influenza are available in some countries and effectively prevent and treat the illness. There are two classes of such medicines, 1) adamantanes (amantadine and remantadine), and 2) inhibitors of influenza neuraminidase (oseltamivir and zanamivir). Some influenza viruses develop resistance to the antiviral medicines, limiting the effectiveness of treatment. WHO monitors antiviral susceptibility in the circulating influenza viruses.

What is WHO doing to facilitate production of influenza A (H1N1) vaccines?

As soon as the first human cases of new influenza A (H1N1) infection became known, the WHO Collaborating Center in Atlanta (The Centers for Disease Control and Prevention [CDC] in the United States of America) took immediate action and began work on developing candidate vaccine viruses. WHO also initiated consultations with vaccine manufacturers worldwide to facilitate the availability of all necessary material to start production of a vaccine. WHO is also working with national regulatory authorities to ensure that the new influenza A (H1N1) vaccine will meet all safety criteria and be made available as soon as possible.

How quickly will influenza A (H1N1) vaccines be available?

The first doses of influenza A (H1N1) vaccine could be available within five to six months after identification of the new virus. Prior to this date, small quantities of experimental vaccine will be made available to immunize volunteers in clinical trials to select the best vaccine formulations. Regulatory approval for the vaccines will be conducted by national authorities in parallel with the manufacturing process. National regulatory authorities have put into place expedited processes that do not compromise on the quality and safety of the vaccine. Delays in production could result from poor growth of the virus strain used to make the vaccine.

So far, it has been impossible to develop vaccines for major killers such as HIV and malaria. How sure are we that there will not be scientific or other hurdles in developing an effective influenza A (H1N1) vaccine?

Typically, development of influenza vaccines has not posed a problem. Influenza vaccines have been used in humans for many years and are known to be safe and effective. Effective vaccines are produced each season. However, one key unknown is the yield of vaccine virus production, since some strains grow better than others and the behaviour of the new influenza A (H1N1) strain in manufacturers’ systems is not yet known. New recombinant technologies are under development, but have not yet been approved for use.

How can a repeat of the 1976 swine flu vaccine complications (Guillain-Barré syndrome) experienced in the United States of America be avoided?

Guillain-Barré syndrome is an acute disorder of the nervous system. It is observed following a variety of infections, including influenza. Studies suggest that regular seasonal influenza vaccines could be associated with an increased risk of Guillain-Barré syndrome on the order of one to two cases per million vaccinated persons. During the 1976 influenza vaccination campaign, this risk increased to around 10 cases per million vaccinated persons which led to the withdrawal of the vaccine.

Is it safe to eat pork / meat and pork products?

Avoiding is better but yes can eat. Influenza A(H1N1) has not been shown to be transmissible to people through eating properly handled and prepared pork (pig meat) or other products derived from pigs. The influenza A(H1N1) virus is killed by cooking temperatures of 160°F/70°C, corresponding to the general guidance for the preparation of pork and other meat.

What can I do to protect myself from catching influenza A(H1N1)?

The main route of transmission of the new influenza A(H1N1) virus seems to be similar to seasonal influenza, via droplets that are expelled by speaking, sneezing or coughing. You can prevent getting infected by avoiding close contact with people who show influenza-like symptoms (trying to maintain a distance of about 1 metre if possible) and taking the following measures:

· avoid touching your mouth and nose;

· clean hands thoroughly with soap and water, or cleanse them with an alcohol-based hand rub on a regular basis (especially if touching the mouth and nose, or surfaces that are potentially contaminated);

· avoid close contact with people who might be ill;

· reduce the time spent in crowded settings if possible;

· improve airflow in your living space by opening windows;

· practise good health habits including adequate sleep, eating nutritious food, and keeping physically active.

What about using a mask? What does WHO recommend?

· If you are not sick you do not have to wear a mask.

· If you are caring for a sick person, you can wear a mask when you are in close contact with the ill person and dispose of it immediately after contact, and cleanse your hands thoroughly afterwards.

When and how to use a mask?

If you are sick and must travel or be around others, cover your mouth and nose.

Using a mask correctly in all situations is essential. Incorrect use actually increases the chance of spreading infection.

How do I know if I have influenza A(H1N1)?

You will not be able to tell the difference between seasonal flu and influenza A(H1N1) without medical help. Typical symptoms to watch for are similar to seasonal viruses and include fever, cough, headache, body aches, sore throat and runny nose. Only your medical practitioner and local health authority can confirm a case of influenza A(H1N1).

Should I take an antiviral now just in case I catch the new virus?

No. You should only take an antiviral, such as oseltamivir or zanamivir, if your health care provider advises you to do so. Individuals should not buy medicines to prevent or fight this new influenza without a prescription, and they should exercise caution in buying antivirals over the Internet.

What about breastfeeding? Should I stop if I am ill?

No, not unless your health care provider advises it. Studies on other influenza infections show that breastfeeding is most likely protective for babies - it passes on helpful maternal immunities and lowers the risk of respiratory disease. Breastfeeding provides the best overall nutrition for babies and increases their defense factors to fight illness.

To which antiviral drugs does this influenza virus respond?

There are two classes of antiviral drugs for influenza: inhibitors of neuraminidase such as oseltamivir and zanamivir; and adamantanes, such as amantadine and rimantadine. Tests on viruses obtained from patients in Mexico and the United States have indicated that current new H1N1 viruses are sensitive to neuraminidase inhibitors, but that the viruses are resistant to the other class, the adamantanes.

Could the virus become resistant to oseltamivir and zanamivir?

Resistance can develop to antiviral drugs used for influenza. Therefore, WHO and its partners are monitoring antiviral drug resistance.
